The situation

Ramon was diagnosed with type 2 diabetes at 47. He was told he would need to manage it for life. His existing physician prescribed medication; no plan was offered for what to do at home. He walked into the Wellness Center after a church-organized community health drive.

What we did together

An intake with a credentialed practitioner produced a written plan in plain Tagalog and English. His wife was included from day one. The Wellness Center coordinated with his physician on medication adjustment as readings improved. The household joined a Family Wellness workshop in month three.

How it played out

Monthly readings were tracked. The plan was adjusted twice over the year — once when his work schedule changed, once when his daughter joined the household routine. Medication was reduced under physician oversight as readings improved.
